FOR  PATIENTS ​NEEDING EXTRA CARE at home

Meds@Home is a new way to care for patients needing long-term level care but desire to stay in their home.
Bayshore Pharmacy is approved to provide pharmacy care to these patients and to assist in decreasing hospitalizations and emergency department visits. We know that people who do not take their medications correctly – too much, too little, wrong time, wrong dose – frequently have complications that result in ER visits and hospitalizations. 

Meds@Home is more than prescription delivery. 
Many Medicare insurance plans pay for Meds@Home expanded pharmacy services which include medication convenience packaging with multiple dose blisters per day and free delivery for eligible patients. 
Additional services include 24-hour access to our pharmacists, home infusion delivery if needed, and our professional pharmacy staff will stay in contact with the patient's physicians to monitor any changes to medication regimens or healthcare needs.

Patients who are physically unable to come to the pharmacy may qualify for Meds@Home. If you or a loved one take 3 or more maintenance medications and have difficulty managing cooking, cleaning, or shopping for yourself, Meds@Home may be a solution, Most Medicare D plans provide access to Meds@Home. 

Can I qualify for Meds@Home?

Do you or a loved one:

• Attend adult day care 5 days a week ​OR
• Require Dialysis?

 Do you or a loved one:

• Take 3 or more maintenance medications and
• Have AARP, Prime Therapeutics, Cigna, or Express Scripts Medicare Part D prescription plan and
• Unable to physically go to the pharmacy and
• Have difficulty managing cooking, cleaning, or shopping for yourself?
